Tethered Oral Tissues

The participation of a speech-language pathologist (SLP) or orofacial myologist (COM) with specific training in orofacial complex structure and function with a focus on oral phase feeding and swallowing is crucial piece of the pre- and post-frenectomy care integrated team. The SLP/OM provides supports for guiding oral and facial structural development, oral movement patterns, and transition to optimal functional patterns for oral rest posture, breathing, breastfeeding, chewing, swallowing, and speech sound production as related to new range of motion and muscle patterns following a frenectomy.

Coordination of facial and oral muscle movement patterns is an intricate process that is impacted by cranial nerve function, oral sensory skills, autonomic nervous system regulation for whole body sensory processing, and muscle activation, relaxation, tone, strength, and endurance. To support optimal function post-frenectomy a program of pre-habilitation to gain increased movement and control of oral muscle movement patterns is recommended which flows into post-operative habilitation activities focused on the development of correct function of the orofacial complex that can be maintained over the lifetime. This program is recommended as part of the integrated team approach for optimal outcomes.
